Please add in the notes the names of the individuals using the tickets.
If you want to expedite the purchase of tickets, please pay using the link above and send an e-mail to scott@haseksheroes.org with the form filled out or the following information:
- Number of tickets purchased
- Name used to purchase tickets
- Player Name of who you are attending in support of
- Team Name of Player

Please Note: Award Nominees are free to attend and guest tickets may be purchased for $39.00* via the above instructions.
All nominees are asked to arrive at 5:30pm to walk the Red Carpet and participate in ceremony photographs. The evening will start with a buffet dinner at 6:00pm and then conclude with the awards ceremony starting at 6:45PM.
